System Thinking and Patient Safety

Course Objectives:
- Describe the purpose of human factors in complex care systems as a systems and design based science which seeks to jointly optimise organizational performance and human wellbeing.
- Discuss “myths and misunderstandings” related to human factors in the investigation of patient safety incident in healthcare and challenge outdated anti-systemic language (e.g cause by “ medical error” and practices (e.g the search of “root causes” ).
- Define and describe “new” theory and practice on incident investigation and team based learning from safety events based on modem safety science concepts.
